Pretty straight forward.
Make copies of the worksheet for each student and the question sheet for each teacher.
Put the students in groups and give them the first question. They work together to answer the question plus write a second sentence with a little more detail. Then one member comes to a teacher and gives their answer. At this point in a smaller class or a class that can handle more advanced activities the teacher or ALT will ask a follow-up question or two about their answer before giving them the next passive tense question. The student returns to their group, they answer the question and a new student comes to a teacher with an answer.
For a demonstration I usually have the JTE ask me questions about my hometown.
A good follow up activity would have the students asking similiar questions about the ALT's country, hometown or even where they live in Japan (I don't live in the same city I work in).
