This is a simple self-introduction worksheet for elementary 3rd graders.
There's a box for students to draw themselves and then four speech bubbles to draw their favorite things. I recommended them to draw colours, food, sport, animals, etc. but also let them choose other things if they like. They can of course use colored pencils. They can choose to write the word next to it in Japanese, English or katakana or I can write it for them.
For the name part, they should be learning how to write romanised letters right now so it's a good challenge for them to write their own name but you can definitely help them with this too.
I intended for the students to fill out this worksheet and then use it to introduce themselves to the new ALT that will come. In this case, I hope they can talk to the ALT using the worksheet and then give the ALT the worksheet so the ALT can read them in their own time and can get to know the kids.
