I always give this to my special education students in JHS. It helps boost their reading and writing skills along with their speaking skills!
Once they have finished filling in their answers, I ask each student the questions and they have to answer aloud - in the case of my student who does not speak at all, he points to each answer, so I switch up the order of the questions from what's printed to make sure he understands me.
I usually change up the questions a little bit with every worksheet and keep it pretty easy since my students are pretty low-level. The questions that always stay the same are "what's your name?" and "how are you?" I tend to cycle in "what day is it?", "what's the date today?", "how's the weather?", "what time is it?", "what ___ do you like?", "when is your birthday?", "what's this?" etc. Feel free to edit and change it based on your students' needs!
